Click on the icon to the right of the Login Name or Password field if you have misplaced your login name or password. The login name or password reset instructions will be sent to the email address entered in your profile from your membership application.
NOTE: Message will appear that says "Check box & enter code". After you click on the box, then your code will be displayed. Type the code into the box. Once all fields are completed, then click the SUBMIT button at the top.
